Rising Tide Student Registration 2023-2025

Welcome! Thank you for your interest in registering your child for the Crossway Rising Tide Upper Elementary Group for 4th & 5th graders! Rising Tide will start on September 8, 2024. We will meet on Sundays from 4:00-5:15 pm in the Fellowship Hall. We look forward to getting back together to study God's Word and have fun together!
Please complete the following registration form to help us provide a fun and safe learning environment for our students. If needed, there is also a separate form linked below for families with multiple children for your convenience. All information will be kept confidential between the Rising Tide Director, Crossway Pastors, and select members of the Rising Tide Servant Team (as needed). Paper copies of registration forms are also available if you do not want your responses stored digitally. In the event that I cannot be reached in case of emergency, I recognize that there may be occasions where the child(ren) named above, may be in need of first aid or emergency medical treatment as a result of an accident, illness, other health condition, or injury. I do hereby give permission for employees or volunteers at Crossway Wilmington to seek and secure any needed medical attention or treatment for the child(ren) named above, including hospitalization, if such need arises. In doing, so I agree to pay all fees and costs arising from this action to obtain medical treatment. I give permission for attending physician(s) and other medical personnel to administer any needed medical treatment, including pain management, anesthesia, and surgery. Photo Policy & Release

Crossway Wilmington will not post photos displaying the faces of Rising Tide students on social media or our website to protect the privacy of our students. With parental consent and for the purposes of promoting/celebrating the ministry, we may use less identifiable photos of the students taken from rear or side profiles so the student’s faces are not shown. If there is a special occasion where we want to post photos on the screen in the Sanctuary showing the children's faces, we will ask each parent for written consent prior to displaying your child(ren)'s photo.
